AMH value: what anti-Müllerian hormone says about the egg supply

AMH, in full anti-Müllerian hormone, plays an important role, according to the NVKC, in women after puberty in the growth and maturation of follicles, the sacs in which eggs mature. The support cells around the small follicles make it, which is why according to the NVKC the amount of AMH in the blood is a measure of the follicle supply.

The amount of AMH falls in women from a certain age, and in polycystic ovary syndrome (PCOS) it is strongly raised. Only a blood test shows how much AMH is in the blood.

Star-SHL counts an AMH between 0.71 and 7.59 µg/l as normal for a woman aged 30 to 34. From 35 to 39, Star-SHL uses 0.41 to 6.96 µg/l.

Turnaround up to 72 hours, estimate Turnaround: up to 72 hours after your blood draw (estimate).

AMH is a measure of the follicle supply in the ovaries, and in women the value falls with age. Star-SHL, Unilabs, Certe and Diagnostiek voor U use limits per age for women.

  • For AMH, Star-SHL uses 1.52 to 9.95 µg/l for women aged 20 to 24 and 0.06 to 4.44 µg/l for those aged 40 to 44.
  • The Dutch guideline for the basic fertility work-up (the first tests for a couple having trouble conceiving) advises against measuring AMH in that work-up.
  • Hormonal contraception can lower AMH: in one study, AMH in users was on average about a quarter lower than in non-users of hormonal contraception, and that effect proved reversible.

AMH does not depend on the menstrual cycle and can be measured on any cycle day. The reference group did not use hormonal contraception: combined contraception lowers AMH, so a value taken while on the pill can come out low. The lower limit of 0.010 at 45-50 years is the detection limit of the test, not a measured percentile. A hormone reference value is assay-specific; these values apply to the Roche Elecsys method.

AMH (Anti-Müllerian Hormone): what this test measures

The AMH test measures the amount of anti-Müllerian hormone in the blood, in micrograms per litre (µg/l). According to the Dutch guideline for the basic fertility work-up (the first tests for a couple having trouble conceiving), AMH relates best to the number of small antral follicles, the follicles that can be counted on ultrasound.

Laboratories use limits per age group for women; Unilabs, for example, gives 0.03 to 5.47 µg/l for women aged 40 up to and including 44. Laboratories also print limits for men, such as Star-SHL's 1.43 to 11.60 µg/l from 18 to 50.

AMH (Anti-Müllerian Hormone): why this value matters

According to the Dutch guideline for the basic fertility work-up (the first tests for a couple having trouble conceiving), AMH predicts fairly well how the ovaries respond to hormone stimulation, as in IVF. The European IVF guideline recommends AMH or an antral follicle count to predict a high or low response to stimulation.

For the chance of a natural pregnancy, AMH appears not to be a good measure. According to the Dutch guideline for the basic fertility work-up, the only study that examined this prospectively shows that AMH does not predict that chance in unexplained reduced fertility. In women without fertility problems, too, AMH appears not to predict it according to that same Dutch guideline. The guideline advises against measuring AMH in the basic fertility work-up.

On AMH in polycystic ovary syndrome (PCOS) the guidelines differ. The 2023 international PCOS guideline says AMH could be used in adults to define polycystic ovaries, but not as the only test for the PCOS diagnosis. The Dutch 2023 adaptation of the 2018 international guideline and the NVKC keep to an ultrasound next to the AMH result.

AMH (Anti-Müllerian Hormone): how to prepare

Nothing special is needed for these tests.

  1. Your cycle does not matter here

    Good to know

This value barely changes, including through a menstrual cycle. Any day works.

  • Water is always fine. Drink as usual beforehand, it makes the draw easier.
  • Take your usual medication, unless your own doctor told you otherwise.

AMH (Anti-Müllerian Hormone): when is testing worthwhile?

According to the NVKC, AMH is measured among other things for insight into the egg supply when premature ovarian insufficiency is suspected, and in investigations of polycystic ovary syndrome (PCOS). In the European guideline on premature ovarian insufficiency, that condition means ovarian activity is lost before the age of 40. In IVF (in vitro fertilisation), AMH helps predict the response to stimulation according to the European IVF guideline.

According to the international PCOS guideline, AMH may vary across the menstrual cycle. Hormonal contraception can also lower the value, so its use counts when reading the result.

AMH (Anti-Müllerian Hormone): symptoms of a high or low value

Low Levels

A low AMH fits a smaller follicle supply, which happens in women from a certain age. After menopause, AMH is lower than 0.1 µg/l according to the NVKC.

A low AMH can also be linked to premature ovarian insufficiency, which the European guideline defines as the ovaries stopping before age 40, to surgery on an endometriosis cyst, to chemotherapy in women of reproductive age with breast cancer or to hormonal contraception.

High Levels

A strongly raised AMH can fit polycystic ovary syndrome (PCOS) according to the NVKC, in which the ovaries hold many small follicles. According to the NVKC, the AMH result must be combined with an ultrasound for the diagnosis. The 2023 international PCOS guideline allows AMH or an ultrasound in adults to establish polycystic ovaries, but AMH alone does not make the PCOS diagnosis.

With a predicted high response to IVF stimulation, the European guideline gives a conditional recommendation for a lower hormone dose to reduce the risk of overstimulation, an excessive response of the ovaries; AMH helps predict such a high response.

AMH (Anti-Müllerian Hormone): causes of a low or high value

What lowers the value

  • Age

The amount of AMH falls in women from a certain age, while FSH rises.

  • Premature ovarian insufficiency

In premature ovarian insufficiency, in which, according to the European guideline, ovarian function stops before age 40, AMH can be low. According to the European guideline, an AMH test may help confirm that diagnosis when the FSH result is inconclusive, but it is not the primary test.

  • Hormonal contraception

Users of hormonal contraception had on average about a quarter lower AMH values in one study than women not using hormonal contraception; that effect proved reversible.

  • Surgery on an endometriosis cyst

After removal of an endometriosis cyst, AMH fell on average in a review of studies, which suggests a possible negative effect on the egg supply, although the studies differed widely.

  • Chemotherapy for breast cancer

In women of reproductive age with breast cancer, AMH fell to undetectable levels during chemotherapy in most and generally stayed very low afterwards in most.

What raises the value

  • Polycystic ovary syndrome (PCOS)

In women with PCOS, AMH is strongly raised according to the NVKC, and according to the NVKC the result must be combined with a transvaginal ultrasound, an internal scan through the vagina, for the diagnosis.

AMH (Anti-Müllerian Hormone): lifestyle and this value

Compare an AMH result with the range for one's own age group, because the same value can fall below the range in one group and within it in another. An AMH of 0.5 µg/l lies below the Unilabs range of 0.89 to 9.85 µg/l for women aged 25 up to and including 29. The same 0.5 µg/l lies within the 0.01 to 2.71 µg/l that Unilabs uses for women aged 45 up to and including 49.

AMH (Anti-Müllerian Hormone): frequently asked questions

What is a normal AMH value?

A normal AMH value depends on age and laboratory. Star-SHL uses 1.20 to 9.05 µg/l for women aged 25 to 29, and Unilabs 0.15 to 7.49 µg/l for women aged 35 up to and including 39.

Does AMH predict whether I can get pregnant?

Probably not: according to the Dutch guideline for the basic fertility work-up (the first tests for a couple having trouble conceiving), AMH appears not to predict the chance of a natural pregnancy in women without fertility problems. That guideline advises against measuring AMH in that first work-up. AMH does say something about the follicle supply and about the response to IVF stimulation.

Does the pill lower my AMH?

Yes, the pill can lower AMH. In a large study, AMH in users of the combined pill was on average about 24 percent lower than in women without contraception.

What does a high AMH mean?

A high AMH can point to polycystic ovary syndrome (PCOS), in which AMH is strongly raised according to the NVKC, although AMH alone does not make that diagnosis. In IVF (in vitro fertilisation), a high AMH helps predict a strong response to hormone stimulation according to the European IVF guideline.
